There is a plugin called WP Touch that works well. You either search for it or I think its on the first page of popular plugins.

There are a number of plug-ins that you can add to any WordPress site that will provide Social Media integration for your site. One that I use is called Sociable. You can also us a service like HootSuite.com to automatically post to social media every time you publish a new post on your site.
