Newbie - Sidebar opt in

I'm just setting up my first socrates site, I was wondering if anyone can advise the best way to set up an opt in form in the sidebar, I'm using 1shopping cart for my Auto Responder?

Also how do you include an image to represent your free give away/report like the one on this socrates site;

On that site the picture was part of the form creation within Aweber. I am not familiar with 1shoppingcart's autoresponder feature but it should provide you with the HTML for the opt-in form. You would place this HTML in a text widget in your sidebar. If they don't include a way in their form designer to include an image you will need to put the HTML to display the image into the same textbox before the form code. Google "html img tag" if you are unfamiliar with the HTML for displaying an image.
